feat: add claude-flow MCP configuration

Add .claude/settings.json to enable claude-flow MCP in all worktrees.

This configuration includes:
- SessionStart hook to auto-start claude-flow daemon
- Auto-approval for claude-flow MCP tools and CLI commands
- Basic claude-flow settings
